RESERVATION FORM FOR APRIL 23, 2004 ICA MEETING

Reservation Deadline: March 31, 2004

Both members and nonmembers, that qualify for membership, can attend this meeting. Bylaws state that any individual, partnership, or corporation engaged in the production of paper converted products in North America and whose sales do not exceed 50 million dollars is eligible for membership in the Association.

We must have your reservation in hand by March 31st non-members must enclose the $40.00 per person fee, with their reservation. There is no fee for members
